Below are the Registration Materials and Waiver required for camp registration. These completed forms for each child must be brought to the first day of camp.

SUMMER CAMP FORMS

Download the Registration Form and bring your child's completed registration forms on Monday morning. We must have a completed registration form for every child attending camp. Please don't combine siblings on the same form.

Download the Participation Waiver and bring your child's completed registration forms on Monday morning. We must have a completed Participant Waiver form for every child attending camp. Please don't combine siblings on the same form.

Allergies or Health Conditions: Please contact us ASAP if your child has any health issues that we should be aware of. If your child needs to take medication during the camp day, please complete the Medication Authorization form and bring it along with your Registration Forms and Waiver. Mentors must hold and dispense all medications.
